Carbetocin is indicated in prophylaxis of uterine atony and excessive bleeding after caesarean section.
Drugs acting on the Uterus
Carbetocin is a drug used to control postpartum hemorrhage, bleeding after giving birth. It is an analogue of oxytocin, and its action is similar to that of oxytocin; it causes contraction of the uterus. Carbetocin binds to oxytocin receptors present on the smooth musculature of the uterus, resulting in rhythmic contractions of the uterus, increased frequency of existing contractions, and increased uterine tone. The oxytocin receptor content of the uterus is very low in the non-pregnant state, and increases during pregnancy, reaching a peak at the time of delivery.
Adult: Single dose of 100 mcg given over 1 min after delivery intravenously.
No specific drug interactions have been reported with carbetocin.
Do not administer before delivery. Vascular disease esp coronary artery disease; hepatic or renal disease. Monitor BP in eclampsia and preeclampsia for up to 8 hrs. Inadequate uterine contraction (do not repeat treatment). Epilepsy; migraine; asthma.
Common side effects are nausea, vomiting, abdominal pain, pruritus, flushing, feeling of warmth, hypotension, headache, tremor, back pain, dizziness, metallic taste, anaemia, sweating, chest pain, dyspnoea, chills, tachycardia, anxiety.
Carbetocin is not indicated during pregnancy, prior to the delivery of the infant. Use of carbetocin during pregnancy could result in hyperstimulation of the uterus with hypertonic or tetanic contractions, tumultuous labor, uterine rupture, cervical and vaginal lacerations, postpartum hemorrhage, uteroplacental hypoperfusion and deceleration of the fetal heart rate, fetal hypoxia, hypercapnea, or death. Carbetocin, in small amounts, has been distributed in the breast milk. This small amount found in breast milk or colostrum after a single injection would not be expected to present a significant safety concern.
Overdosage of carbetocin can be expected to produce enhanced pharmacological effects associated with uterine hyperactivity and pain. Treatment consists of symptomatic and supportive management.
Store at 2-8°C. Do not freeze. Keep away from light. Once the ampoule has been opened, the product should be used immediately.
