Povidone-iodine is a broad spectrum antiseptic for topical application in the treatment and prevention of wound infection. It may be used in first aid for minor cuts, grazes, burns, abrasions and blisters.
Povidone Iodine is a complex of iodine and an organic polymer, povidone. This polymerization makes Povidone Iodine superior to ordinary elemental iodine. It prolongs the germicidal activity of iodine by liberating elemental iodine slowly. Consequently it has a lower toxicity than elemental iodine. It gives rapid microbicidal activity against both Gram-positive and Gram-negative bacteria, protozoa, viruses and fungi/yeasts. It is also sporicidal. It is the only microbicide with this broad spectrum of activity. It is non-staining, exerts prolong germicidal action and is also active in the presence of soap, blood, serum, pus, mucosal secretions and water.
MOUTH/THROAT Oral hygiene: Adult: Rinse mouth w/ 10 mL of a 0.1% undiluted oc diluted soln w/ an equal amount of warm water soin for 30 sec qid continued for 14 days. Child: >6 yr: Same as adult dose. Oral candidiasis: Adult: Rinse mouth w/ 1% mthwsh solution. TOPICAL/CUTANEOUS Treatment & prophylaxis of wound infection: Adult: Apply 0.5-5% topical powders on affected area.
It exhibits interaction with strong alkali, sodium thiosulphate, sodium metabisulphite and thiomersal. Use with concurrent lithium therapy has been shown to exhibithypothyroidic effect.
Hypersensitivity; prolonged use in patients w/ thyroid disorders or on lithium therapy. Premature neonates or neonates weighing <1.5 kg. Avoid contact w/ eyes; should not be used under occlusive dressing. Pregnancy, lactation; neonates.
Local irritation & sensitivity (rare). Application to large areas of denuded skin may produce systemic effects due to iodine absorption.
The use of povidone-iodine in the mother near term and during breastfeeding increases breastmilk iodine levels and can cause transient hypothyroidism in breastfed infants, especially in geographic areas that are iodine deficient. Maternal exposure to povidone-iodine near term can sometimes interfere with thyroid studies done as a part of newborn screening tests. Although iodine from povidone-iodine is minimally absorbed through intact adult skin, exposure of mothers who are or will be breastfeeding to povidone-iodine should be minimized by using lower concentrations of povidone-iodine, applying it to the smallest possible surface areas of the body, shortening contact time, and avoiding repeated applications. Iodine absorption can be extensive with vaginal use; avoid douching with povidone iodine or use of iodine-containing tampons during breastfeeding. Animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in humans, but potential benefits may warrant use in pregnant women despite potential risks.
