Selective Serotonin Re-uptake Inhibitor; Antidepressant
Fluvoxamine blocks the reuptake of serotonin at the serotonin reuptake pump of the neuronal membrane, enhancing the actions of serotonin on 5HT1A autoreceptors. Studies have also demonstrated that fluvoxamine has virtually no affinity for α1- or α2-adrenergic, β-adrenergic, muscarinic, dopamine D2, histamine H1, GABA-benzodiazepine, opiate, 5-HT1, or 5-HT2 receptors, despite having an affinity for binding to σ1 receptors.
ORAL Social anxiety disorder: Adult: As modified-release preparation: Initial: 100 mg once daily. Max: 300 mg daily.
Fluvoxamine maleate should not be used concurrently with monoamine oxidase inhibitors, warfarin, benzodiazepines, theophylline, aminophylline, tricyclic antidepressants and alcohol.
Hypersensitivity. Not to be used w/ thioridazine, terfenadine, astemizole, cisapride, pimozide, aloestron, tizanidine. Lactation. History of mania or seizures; liver dysfunction; presence of depressive symptoms; smokers. Treatment w/ MAOI should only be started at least 2 wk after stopping fluvoxamine treatment. Increased risk of suicidal ideation & behaviour when used in children, adolescents & young adults <24 yr. Pregnancy, elderly; operating hazardous machinery; wldraw gradually. Monitor prothrombin time in patients who are taking oral anticoagulants concurrently.
Headache, asthenia, tremor, palpitations, nausea, diarrhoea, constipation, anorexia, vomiting, flatulence, somnolence, insomnia, dry mouth, nervousness, dizziness, tremor, anxiety, agitation, decreased libido, depression, CNS stimulation, dyspnoea, yawn, sweating, abnormal ejaculation, urinary frequency, anorgasmia, urinary retention.
This drug should be used during pregnancy only if the benefit outweighs the risk to the fetus. A decision should be made to discontinue breastfeeding or discontinue the drug, taking into account the importance of the drug to the mother.
Symptoms: In case of overdosage the symptoms include gastro-intestinal complaints (nausea, vomiting and diarrhea), somnolence and dizziness have been seen. Cardiac events (tachycardia, bradycardia, hypotension), liver function disturbances, convulsions, and coma have also been reported. Treatment: There is no specific antidote to fluvoxamine. In case of overdose the stomach should be emptied as soon as possible after tablet ingestion and symptomatic treatment should be given. The repeated use of medicinal charcoal if necessary, accompanied by an osmotic laxative, is also recommended.
